1. Introduction 1. The health status of hundreds of millions of people in the world today is unacceptable. More than half the population of the world does not have the benefit of adequate health care. There is a wide gap between the developed and developing countries in their levels of health and in the resources they are devoting to the improvement of health. Moreover, within individual countries, whatever their level of development, analogous gaps are commonly evident between different groups of the population. 2. The Constitution of WHO 'and numerous Health Assembly resolutions have reaffirmed that health is a basic human right and a worldwide social goal; that it is essential to the satisfaction of basic human needs and the quality of life; and that it is to be attained by all people. In 1977 the Thirtieth World Health Assembly decided, in resolution WHA30.43, that the main social target of governments and WHO in the coming decades should be the attainment by all citizens of the world by the year 2000 of a level of health that will permit them to lead a socially and economically productive life. 3. The Declaration of Alma-Ata, adopted on 12 September 1978 by the International Conference on Primary Health Care, which was jointly sponsored and organized by WHO and UNICEF, clearly stated that primary health care is the key to attaining the target of health for all by the year 2000 as part of overall development and in the spirit of social justice. The Declaration called on all governments to formulate national policies, strategies and plans of action to launch and sustain primary health care as part of a comprehensive national health system and in coordination with other sectors.
