Excerpt: Certification Statement Instructions: The individual responsible for directing or supervising the preparation of the enclosed Industry Short Technical Questionnaire: Phase II Cooling Water Intake Structures must read and sign the Certification Statement below before returning both documents to the U.S. Environmental Protection Agency. The certifying official must be a responsible corporate official or his (or her) duly authorized representative.
