In the Seventies WHO redirected its a t t e n t i o n on the health care system from highly trained s p e c i a l i s t care to what was termed primary h e a l t h c a r e . The o f f i c i a l expression of t h i s reorientation was the Declaration of ALMA ATA. The discussion of the goal Bealth For A l l by the Year 2000 has made it c l e a r t h a t WHO must go yet one s t e p f u r t h e r and take into consideration not only a reorganization of medical care in the d i r e c t i o n of the cornunity but a l s o recognize the importance of the health related a c t i v i t i e s and health beliefs of the community i t s e l f , the relevance of s o c i a l support systems, c u l t u r a l l y defined l i f e s t y l e s and human ecology for the quality of l i f e and the maintenance of health. It i s not only the problems of financing f u r t h e r growth in the s e r v i c e s e c t o r s t h a t have led to a new i n t e r e s t i n countries in the lay system of health care in the hope of unpaid service provision through families and voluntary work) but a l s o a growing uneasiness with i n s t i t u t i o n a l care, doubts about certain aspects of professional service provision and loss of c o n v i v i a l i t y in a l l spheres of daily l i f e .
